MOTIVATION: Any type of casualty can occur on the first day of a contingency operation. Therefore, casualty managers from each echelon of command must deploy without delay. The Defense Casualty Information Processing System (DCIPS) tracks casualty data pertaining to Service-members, Family Member, Civilians, and other interested persons for Army, Navy, Marines, and Air Force. DCIPS is the automated migration system for the casualty and mortuary affairs process within the DoD. The mission of DCIPS is to record, report, verify, and process casualty information from unit level to HRC; notify appropriate individuals; and provide casualty assistance to next of kin as required by regulatory guidelines.

2. Terminal Learning Objective (TLO)

NOTE: Inform students of the following Terminal Learning Objective (TLO) requirements.

SAFETY REQUIREMENTS. In a training environment, leaders must perform a risk assessment in accordance with FM 5-19, Composite Risk Management. Leaders will complete a DA Form 7566 COMPOSITE RISK MANAGEMENT WORKSHEET during the planning and completion of each task and sub-task by assessing mission, enemy, terrain and weather, troops and support available-time available and civil considerations, (METT-TC). Note: During MOPP training, leaders must ensure personnel are monitored for potential heat injury. Local policies and procedures must be followed during times of increased heat category in order to avoid heat related injury. Consider the MOPP work/rest cycles and water replacement guidelines IAW FM 3-11.4, NBC Protection, FM 3-11.5, CBRN Decontamination. No food or drink is allowed near or around electrical equipment (CPU, file servers, printers, projectors, etc.) due to possible electrical shock or damage to equipment.  Exercise care in personal movement in and through such areas.  Avoid all electrical cords and associated wiring.  In event of electrical storm, you will be instructed to power down equipment. Everyone is responsible for safety.

RISK ASSESSMENT LEVEL. Low.

ENVIRONMENTAL STATEMENT. Environmental protection is not just the law but the right thing to do. It is a continual process and starts with deliberate planning. Always be alert to ways to protect our environment during training and missions. In doing so, you will contribute to the sustainment of our training resources while protecting people and the environment from harmful effects. INSTRUCTIONAL LEAD-IN. DCIPS tracks casualty data pertaining to service members and their families. DCIPS is the automated migration system for the casualty and mortuary affairs process within the Department of Defense.

3. Introduction to DCIPS

1. Learning Step / Activity 1. Introduction to Defense Casualty Information Processing System (DCIPS)

a. DCIPS provides casualty, mortuary affairs, personal effects tracking and processing, remains tracking, line of duty (LOD), and Freedom of Information Act (FOIA) management capability for casualties from current and prior conflicts. DCIPS- Case Management (DCIPS-CM) is fed by DCIPS-Forward (CF) and DCIPS-Casualty Reporting (CR) and updated at HRC level using verifying documents along with supplemental (SUPP) reports from reporting Casualty Assistance Centers (CACs).

b. DCIPS provides the field user with the capability to collect and report casualty data to a Service headquarters or higher reporting unit; it is typically used by units that do not have NIPRNET access. The field users has access to two DCIPS components:

  1. DCIPS-Forward (DCIPS-CF) 8.0 - Stand-alone for remote users.
  2. DCIPS-Casualty Reporting ( DCIPS-CR) - Web application.
c. DCIPS is used by all services to send casualty reports electronically and at Casualty and Mortuary Affairs Operations Center (CMAOC) level to maintain casualty information; such as information pertaining to LOD, decedent's next of kin information, burial, mortuary and memorial affairs, and information about the disposition of the decedent's remains.

NOTE: Inform students that DCIPS-Casualty Forward (DCIPS-CF) 8.0 went live on 13 September 2011

4. Casualty Report Submission Flow

d. Casualty reports may be loaded to DCIPS-CR by a designated reporting unit or directly to DCIPS-CM at your Service headquarters depending on unit capabilities and Service directed procedures.

e. There are three basic ways a casualty report may be submitted to your Service headquarters' casualty office and DCIPS-CM:

(1) DCIPS -Casualty Forward (DCIPS-CF) - the stand-alone casualty reporting tool used by units in the field to report casualty information to higher headquarters . There are two types of DCIPS-Forward casualty reports:

(a) Hasty Casualty Report – A Hasty Report includes only the essential information needed to send an initial or supplemental casualty report.

(b) Full Casualty Report – A Full Report includes detailed information about the casualty (e.g., incident data, personnel data, body armor, awards, progress reports, CAO and NOK data).

(2) DCIPS - Casualty Reporting (DCIPS-CR) – web-based casualty reporting tool used by units and higher echelons to report information higher. It can import Hasty or Full Reports from DCIPS-CF.

(3) DCIPS Case Management (DCIPS-CM) – is used by the Service headquarters to manage casualty cases. DCIPS-CF and DCIPS-CR feed DCIPS-CM with casualty information - hasty and full reports respectively.

NOTE: Inform the students that they can send a Full or Hasty report to Service Headquarters using DCIPS-Forward (DCIPS-CF). DCIPS-CF Hasty or Full report may be emailed to any Casualty Service Headquarters and imported into DCIPS-CR then submitted to DCIPS-CM through the DCIPS-AL (Autoload). Reports generated in DCIPS-CF cannot be directly imported into DCIPS-CM without DCIPS-AL.

2 .Learning Step / Activity 2. Demonstrate Functions of the Defense Casualty Information Processing System – Casualty Forward (DCIPS-FWD).

a. DCIPS-FWD version 8.0 is the replacement for all previous versions. DCIPS-FWD is available for download from:

(1) DCIPS web portal on NIPRNET or

(2) AKO/DKO:(https://www.us.army.mil/suite/kc/6566830) or AKO: Files; Quick View Files ID 32642930; and User Guide File ID 32642856.

(a) Select DCIPS Forward 8.0 Download Folder;

(b) Select DCIPS Forward 8.0.066 Application File (mdb);

(c) Copy the DCIPS-FWD.MDB file to any location on your PC where you have rights, (e.g., “My Documents”). Users may rename the MDB file to any valid file name;

(d) Download the DCIPS-Forward Version 8.0 User's Guide;

(e) If using a common drive, select the drive and file location, then save. Download both the application and the User's Guide to the file on the common drive; close net browser. (For users with a common drive file location: open file, right click on application (mdb), and send to desktop. For individual users with no common drive: download to preferred file location and set shortcut on desktop.)

NOTE: Tell the students not to copy the DCIPS Forward 8.0 MDB file to an existing DCIPS-Forward folder. Always copy the file to a new location to ensure the user does not accidentally overlay existing data in a previous version.

NOTE: Inform the students that each time they start DCIPS Forward they must enable macros. DCIPS Forward will not run unless they enable blocked expressions and/or enable macros.

b. Multiple users can simultaneously access DCIPS-FWD when the DCIPS-FWD.MDB file is on a shared network drive (not to exceed five simultaneous users). Each user can then run DCIPS-FWD from the same location and update the same database. Users will get record lock errors when two or more people are in the same record at the same and attempt to make conflicting changes. When a conflict occurs a message will be displayed. Make sure to read the error message carefully to choose the correct response. When two or more people are in the same record at the same time and one person makes a change, the change will be reflected for each of the other users in that record.

8. Major Changes DCIPS-Forward 8.0

c. DCIPS-Forward (DCIPS-CF) Version 8.0 simplifies the process to collect, report, and monitor casualty reports by units, higher echelons, and Service Headquarters. DCIPS-CF Version 8.0 features:

(1) Corrected some import errors for some systems.

(2) Adds back the old .rsf attachment as part of the e-mail message.

(3) Reports may be saved either using the whole message .txt or just save the .rsf file and import.

(4) Eliminates the requirement to attach .rsf or .txt files to an email message.

9. DCIPS Forward Main Menu

d. DCIPS_CF 8.0 Main Menu consists of the following choices:

(1) Hasty Report Option: Allows the user to quickly enter a casualty report with only the minimal essential information. A Hasty report may be used for individual or multiple casualty incidents. The 17 Required fields are highlighted in blue. The report is submitted to higher as an initial report.

(2) Multiple Casualty Incident Option: Allows the user to create, edit, and assign casualties to multiple casualty incident templates.

(3) Casualty Main Listing Option: Is a queue of current casualty reports and a dashboard from which to create full casualty reports, edit, export, import, and email casualty reports.

(4) Archive Maintenance Option: Allows the user to archive casualty records and remove them from the user's list of casualties on the Casualty Listing Form. Casualty records that have been closed are moved to the Archive Maintenance screen.

(5) Database Administration Option: The Database Maintenance Form provides four basic functions to manage the user's database:

(a) Copy the database.

(b) Upgrade to a new version of DCIPS-Forward.

(c) View lookup codes used to populate data choices.

(d) Import New codes to update lookup codes in your copy of DCIPS- Forward.

(6) Management Reports Option: Provides the ability to print a list of casualty reports. Reports can also be exported and saved as a Microsoft Excel spreadsheet.

NOTE: Inform the students that the bottom of the Main Menu displays the version number and release date.

10. DCIPS Forward Casualty Main Listing

e. The Casualty Main Listing option takes the user to the Casualty Listing Form which lists all of the user's casualty reports and provides the ability to edit, export/import, and email casualty reports.

(1)The queue is the upper portion of the Casualty Main Listing Screen is divided into sections to display casualty reports stored in the database by: report number, report type, incident date, last sent, SSN, name, report version (Full or Hasty).

(2) Casualty Main Listing Screen Features:

(a) Sorting the List of Casualties - From the dashboard features of the Casualty Main Listing Screen, the list of casualties may be sorted by Report #, Incident Date, or Last Name by selecting the appropriate radio buttons.

(b) Creating a New Casualty Record - Select the New button to create a new casualty record

(c ) Editing a Casualty Record - Select the Edit button to edit a casualty report you have already created

(d) Emailing a Casualty Report - Select the Email (Outlook) push button to create an email casualty report for the currently selected casualty

(e) Email Content - Select the Email (Content) push button to create a casualty report that you can copy and paste into an email product other than Microsoft Outlook.

(f) Replicating a Casualty Record - Select the Replicate push button to replicate a casualty report. Use this function to create a Supplemental (SUPP), Status Change (STACH) ,or Progress (PROG) report for a casualty

(g) Deleting a Casualty Record - Select the Delete push button to delete the selected casualty report. Use caution with this feature because the delete cannot be undone.

(h) Exporting a Casualty Record - Select the Export push button to export the selected casualty report. Use this function to create an extract file that can be used to import into another copy of DCIPS-Forward or DCIPS-CR.

(i) Importing a Casualty Record - Select the Import push button to import a casualty record. You may import a file that has been exported from another copy of DCIPS-Forward.

(j) Searching for a Specific Casualty Record - Search for a specific casualty record by entering either a SSN or a Last Name in the fields in the Search group box on the Casualty Listing Form.

11. DCIPS Forward Casualty Information Form

NOTE: Inform the student that the Casualty Information Form displays the Casualty, Incident, Personnel Information, Secondary, Body Armor, Awards, Progress Report, CAO, and NOK tabs. These tabs will not appear if the report they are editing is a Hasty report, they will only see the Hasty Report screen.

f. The Casualty Information form is composed of the following tabs:

(1) Casualty Tab

(2) Incident Tab

(3) Personnel Information Tab

(4) Secondary Tab

(5) Body Armor Tab

(6) Awards Tab

(7) Progress Report Tab

(8) Casualty Assistance Officer (CAO) Tab

(9) Next of Kin (NOK) Tab

NOTE: Inform the students that each of the tabs listed on the Casualty Information Form will be explained in further detail during the block of instruction on “Creating a Full Casualty Report” .

12. Creating a Hasty Casualty Report

g. A Hasty Report is usually the first report generated and contains only essential information about the casualty.

13. DCIPS-FWD Main Menu Splash Screen

NOTE: Inform the students that from the Main Menu, select the “Hasty Report” main menu option to create a Hasty casualty report.

Follow these steps to create a Hasty casualty report:

1. From the Main Menu press the Hasty Report main menu option.

2. Enter the casualty information on the form.

3. Press the “save“ and then the “close” push button. You are returned to the Main Menu.

14. Hasty Report

(1) The light blue data fields indicate the required items that must be completed to send a Hasty report. Reports without these items will not email. The Hasty provides the necessary information on a casualty and can be replicated to a Hasty SUPP, PROG, or STACH. Replication for additional reports gives the option for a Full or Hasty. Once a Hasty has been replicated to a Full, it cannot become a Hasty again.

(2) Body Armor must be included in the Circumstances for Army reports. Remarks must contain who prepared the report (DA Form 1156) and field grade officer review (name and position).

(3) Enter the casualty information on the form. As stated earlier, all required fields are highlighted in blue. Report Type - Initial (INIT) is automatically populated when selecting the Hasty report menu option

NOTE: Inform the students that each report must be numbered using an established format. For Initial Reports, FY - # # # # # etc. up to eight characters. For example, 12-1220, refers to report number 1220 in FY 2012. For subsequent reports, use a sequential number as in the example: SUPP01, SUPP02, STACH01, STACH02, PROG01, PROG02, etc. Refer students to AR 600-8-1, Table 4-4, Message item: 04, Report Number, for further information.

(4) When entering large amounts of data in the remarks, circumstances, and cause of death fields, double click to activate ZOOM and Spell Check. This allows for easier entry when entering large amounts of data. Once data entry is complete, click on “Save”, then “Close”. Once closed, the user is returned to the Main Menu.

NOTE: Inform the students that they must return to the Main Menu and select the Casualty Main Listing option if administrative changes/edits to the hasty report are needed.

NOTE: Inform students that they should double click on the circumstance block to ensure this box is annotated correctly and free or errors/misspelled words. Have the students click on save and then close. The Hasty Report will appear in the Casualty Main Listing.

NOTE: Inform the students that they must return to the Main Menu and select the Casualty Main Listing option to make administrative changes/edits to the “Hasty Report” if needed.

17. Hasty Report - Replicate

h. The “Replicate” function allows the user to create a supplemental, status change, or progress reports for a previously submitted Hasty Report. Replicating also gives the user the option to create a Full report from a Hasty report.

Follow these steps to “Replicate” a casualty report:

1. From the Main Menu, select the Casualty Main Listing menu option.

2. From the Casualty Listing Form, select the casualty report to replicate.

3. Select the Replicate button; the Replicate Options dialog is displayed.

4. Select the Hasty button to replicate the current Hasty report to a Hasty supplemental, status change or progress report. Select the Full button to replicate the currently selected Hasty Report to a Full report. The Replicate Options dialog will only appear when the user has selected a Hasty report to replicate.

5. The casualty record selected will be replicated and appear in the casualty list. Only Casualty and Personnel Information are replicated.

6. Select the replicated casualty record and select Edit to enter updated information about the casualty.

7. Submit the updated casualty report.

18. Hasty Report Relicate - Hand On (2 of 2)

19. Creating a Full Casualty Report

i. A Full Casualty Report option provides the ability to add information about the casualty to include next of kin, casualty assistance officer, body armor, awards, progress reports, personnel, and incident information to the casualty report.

Note to Instructor: Inform the students that the instructions they will receive on creating a Full Report within DCIPS-CF, will also apply to creating a Full (New) Report in DCIPS-CR. These instructions “WILL NOT” be repeated in Learning Activity 3. Demonstrate Functions of the Defense Casualty Information Processing System (DCIPS) - Casualty Reporting (DCIPS-CR).

20. DCIPS-FWD Main Menu Splash Screen

(1) From the Individual Casualty Listing Form, click on the “New” button and enter the casualty information on the Casualty Information Dialog tabs: Casualty, Incident, Personnel Information, and Secondary.

Follow these steps to create a Full Casualty Report:

1. From the Main Menu press the Casualty Main Listing main menu option.

2. From the Individual Casualty Listing Form press the “New” push button.

3. Enter the casualty information on the Casualty Information Dialog tabs Casualty, Incident, Personnel Information and Secondary.

4. Press the “Save” push button. The following additional tabs will appear: Body Armor, Awards, Progress Report, CAO, and NOK.

5. Enter additional information as available.

6. Press the “Save” push button.

7. Press the “Close” push button to return to the Casualty Main Listing Form. The casualty you entered will appear in the list of casualties and will be listed as a Full report.

8. To submit the report press the Email (0utlook) or Export push button.

21. Full Report

(2) The Full casualty report has more detailed personnel and NOK information gathered from military personnel records received from the Home Station Casualty Assistance Center, mobilization station or other authorized reliable sources. The light blue fields indicate required items to send a casualty report.

(3) Many of the data fields under the Casualty tab provide drop down menu choices and those that don't are self- explanatory. It is important to note that the field report number should contain up to eight characters. Some numbering format examples are:

(a) Initial = FY##### (indicates the Fiscal Year and numbering system the user chooses).

(b) Supplemental = SUPP01 or S01.

(c) Progress = PROG01 or P01.

(d) Status Change = STACH01.

(4) Special category casualty matters are unique or bizarre circumstances resulting in a casualty that can be expected to generate media interest. Special interest is a casualty incident involving a person subject to special interest. Persons that fall into this category are:

(a) Active duty general officers and general officer designees.

(b) Retired general officers.

(c) Officers commanding battalions/brigades or higher units of similar size and responsibility in hostile fire areas.

(d) Field grade officers in an advisory position in hostile fire areas.

(e) Former or present Sergeants Major of the Army.

(f) Any government official or public figure that becomes a casualty while under Army sponsorship.

(g) Returned prisoners of war.

(h) Other persons who the Director, CMAOC identifies as subject to special interest.

(5) Information under the Incident tab is used to notify the NOK in the event of a casualty. Accuracy and timeliness is critical; therefore, the user must provide as much information as possible.

(6) The Personnel Information tab captures information about the casualty. New data fields have been added to track civilian casualty information (pay grade, employer, contract agency, etc.).

(7) The Secondary tab provides additional personnel information on Reserve Soldiers on duty other than mobilization orders, retiree disability status, death gratuity remarks, and general casualty report remarks.

27. Full Casualty Report-Body Armor Tab

a. The Body Armor tab captures the type of body armor the casualty was wearing when he/she became a casualty. This tab contains a detailed drop down menu of protective gear. There are three types of body armor: Vest, Helmet, and Eye Protection.

Follow these steps to enter body armor for a casualty:

1. Select the Body Armor Tab.

2. Select the Characteristic for the Vest. Depending on what is selected, the Attachment list may populate.

3. If items appear in the Attachment list box, then select the applicable items. You may click on each attachment that is applicable, i.e., you may select more than one item.

4. Select the Characteristic for the Helmet. Depending on what is selected the Attachment list may populate.

5. If items appear in the Attachment list box, select the applicable item. You may only select one item in the Attachment list box for helmets.

6. Select the Characteristic for Eye Protection. There are no choices for Attachments.

7. Press the “Save” push button .

b. The Awards tab captures awards, badges, and other items that the casualty has. Select data fields in the order they are presented: Service, category, award, pending, and posthumous. Pending signifies awards submitted but not yet approved. Posthumous is awards approved and received after the casualty dies. Saved awards appear in the Award Information section. After the report is saved, users can add or delete awards by clicking either the “New” or “Delete” buttons on the bottom center of the screen.

Follow these steps to enter awards for a casualty:

1. Select the Awards Tab.

2. If the Award Information list is empty at the top of the tab, enter the information in the Service, Category, Award, etc. fields. Press “Save”. The award will appear in the Award Information list.

3. If there are existing awards listed in the Award Information list, then press the “New” push button to add another award. The Service, Category, Award, etc. fields will clear out. Enter the information for the award.

c. The Progress Report tab captures information about the medical progress of an injured or ill casualty.

Follow these steps to enter progress reports for a casualty:

1. Select the Progress Reports Tab.

2. If the Progress Report Information list is empty at the top of the tab, enter the information in the Report Date, Hospital, City, State, etc. fields. Press Save. The progress report will appear in the Progress Report Information list.

3. If there are existing progress reports listed in the Progress Report Information list, then press the New push button to add another progress report. The Report Date, Hospital, City, State, etc. fields will clear out. Enter the information for the progress report.

4. Press the “Save” push button. The progress report will be added to the Progress Report Information list.

30. Casualty Assistance Officer (CAO) Tab

d. The Casualty Assistance Officer ( CAO) tab includes information on the CAO assigned to assist the casualty's family. The responsible CAC will SUPP the initial reports with the CAO information to include name, rank, duty station, status, CAC, and all available contact information for the CAO selected.

Follow these steps to enter CAOs for a casualty:

1. Select the CAO Tab.

2. If the CAO Information list is empty at the top of the tab, enter the information in the CAO Last Name, CAO First Name, Suffix, etc. fields. Press “Save”. The CAO will appear in the CAO Information list.

3. If there are existing CAOs listed in the CAO Information list, then press the New push button to add another CAO. The CAO Last Name, CAO First Name, Suffix, etc. fields will clear out. Enter the information for the CAO.

4. Press the “Save” push button. The CAO will be added to the CAO Information list.

31. Next of Kin (NOK) Tab

e. The Next of Kin (NOK) tab includes information for NOK notification. Enter the information for each NOK to be notified and include NOK receiving benefits designated as the Person Authorized to Direct Disposition of remains (PADD) or the Person Eligible to Receive Effects (PERE).

Follow these steps to enter NOKs for a casualty:

1. Select the NOK Tab.

2. If the NOK Information list is empty at the top of the tab, enter the information in the NOK Last Name, First Name, Suffix, SSN, etc. fields. Press “Save”. The NOK will appear in the NOK Information list.

3. If there are existing NOKs listed in the NOK Information list, then press the New push button to add another NOK. The NOK Last Name, First Name, Suffix, SSN, etc. fields will clear out. Enter the information for the NOK.

4. Press the “Save” push button. The NOK will be added to the NOK Information list.

32. Emailing Casualty Reports

NOTE: Tell students that Microsoft Outlook is the email program used and they should have a basic understanding of how to use Outlook.

33. DCIPS-Forward Main Menu Splash Screen

j. To submit a casualty report from DCIPS via Email, select the “Casualty Main Listing” option from the Main Menu.

Follow these steps to create the .rsf file or email content.

1. From the Main Menu press the Casualty Main Listing main menu option.

2. From the Casualty Listing Form, select a casualty.

3. Press the eMail (Content) push button. The eMail (Content) dialog will display.

4. Press the “Copy All” push button to copy the contents to the clipboard.

5. Create a new email message .

6. Paste the contents of the clipboard into the message body of the email.

7. Send the email message using your email send feature.

34. Emailing Casualty Reports (1 of 2)

(1) From the Casualty Listing Form, select the casualty report to be submitted, then click on the “Email (Outlook)” button. DCIPS-FWD will launch Microsoft Outlook and open an email message with the Casualty information.

NOTE: Tell the students that if all of the required casualty information is not entered, a message box will inform them to complete the missing information.

35. Emailing Casualty Reports (2 of 2)

NOTE: Brief the students that the body of the email message can not be altered. Doing so may prevent the casualty message from properly loading into DCIPS. If the user needs to correct any portion of the casualty message, they must discard the email, correct the data in DCIPS-FWD, and regenerate the casualty message.

(2) Once the user has Microsoft Outlook launched and the casualty message is created with the .rsf file attachment, the user will enter the applicable email addressees and sends the message.

(3) Once the receiver receives the email, they can save the .rsf attachment file on the desktop or other location. The .rsf file can subsequently be imported into DCIPS-CF or DCIPS-CR (web-based reporting).

(4) The report can also be exported and sent as an email attachment using alternate email server; save message as .txt.

36. Create Email Content

k. Users can create email content when they do not have Microsoft Outlook (i.e., AKO).

Follow these steps to create Email content:

1. From the Main Menu, click on the Casualty Main Listing main menu option; then from the Casualty Listing Form, select a casualty.

2. Click on the Email (Content) button; the Email (Content) dialog will display.

3. Press the “Select All” button to copy the contents to the clipboard.

4. User creates a new email message from an email server of choice.

5. Select the message body field from email server and paste the contents of the clipboard into the message body.

6. Send email message using email server's send feature.

37. Importing and Exporting Casualty Reports

NOTE: Tell the students that slides 38-39 describe the procedures for importing and exporting casualty reports.

38. Importing a Casualty Report

l. Users can import a casualty report that was created from (exported) a record from another unit's DCIPS-CF.

Import a casualty report by selecting the Casualty Main Listing option on the Main Menu, then follow these steps:

1. Click on the “Import” button on the Casualty Main Listing Form; the Import File dialog will open.

2. Locate the imported .rsf file and select it. (Note: This .rsf file was created via an email from another user or unit's DCIPS-FWD and saved on users desktop or another location.)

3. Click on the “Open” button.

4. The casualty record can be imported into DCIPS-CF and displayed on the Casualty Listing Form or DCIPS-CR and displayed in the “Queue” .

NOTE: Brief the students that the contents of the imported file can not be changed. Doing so may cause data corruption when the file is imported. If the user needs to change the contents of the file, return to DCIPS-FWD, update the information using DCIPS-FWD, and regenerate the file.

39. Exporting a Casualty Report

m. Individual casualty records may be exported to a text file that may then be imported into another DCIPS-CF application or uploaded to DCIPS-CR. The casualty report will be saved to a file with a file extension of .RSF. This is a text file and can be opened with any text editor.

Export a casualty report by selecting the Casualty Main Listing option on the Main Menu, then follow these steps:

1. From the Casualty Listing Form select the casualty report you want to submit.

2. Press the “Export” push button in the Record Transfer group box.

3. From the file “Save As” dialog box, give the file a name, select a location to save the file, and save it. DCIPS-Forward gives the export file name an extension of .RSF by default. You may also use the .TXT extension. You should adopt a standard naming convention for your export files to avoid errors, (e.g., use the service member's last name, first name and last four of his or her SSN). For example: DoeJohn1234.RSF. Once you've named the imported file, click on open and a dialogue box indicating “Export Complete” will appear.

4. The file is now stored in the selected location. Provide this file to your reporting unit that has access to DCIPS-CR. The person with DCIPS-CR access will import this file (i.e., casualty report) into DCIPS-CR and submit it using DCIPS-CR. See your DCIPS-CR online help for additional information on how to import this file into DCIPS-CR.

NOTE: Brief the students that the contents of the imported file can not be changed. Doing so may cause data corruption when the file is imported. If the user needs to change the contents of the file, return to DCIPS-FWD, update the information using DCIPS-FWD, and regenerate the file.

40. Multiple Casualty Incidents

n. The “Multiple Casualty Incident” main menu option allows the user to create, edit, and assign casualties to multiple casualty incidents. Create a multiple casualty incident report by selecting the Multiple Casualty Incident option from the Main Menu, then follow these steps:

1. The Multiple Casualty Listing Form will open. If a multiple casualty incident is already displaying, then click on the “New” button to create a new incident; if there are no previous multiple casualty incidents displayed, proceed to step 2.

2. Enter a Multiple Casualty Code for the multiple casualty incident. Multiple Casualty Codes reflect the two character abbreviation for country or state of incident, fiscal year, and numeric sequence. Multiple Casualty Codes can stay with report from beginning to end. (e.g., AF-12011. AF is Afghanistan; 12 is fiscal year; 011 is the sequence number for the multiple casualty).

3. Enter Multiple Casualty Description for the incident. Create a short unique description of the incident with event, location, and date/time of incident. (e.g. On 19 Jan 12, Soldiers were on a Security patrol at MSR Columbia encountered IED at approximately 0225).

4. Complete the information on the Incident Tab and Incident (cont) Tab as appropriate.

5. Click on “Save” and then “Close”. (NOTE: Once you click “Save” the “Multiple Casualty Incident Code” at the top of the page will populate and the Multiple Casualty Code and Multiple Casualty Description boxes become unavailable for editing.

41. Multiple Casualty Incidents

o. There are three (3) tabs as part of the Multiple Casualty Incident form: Incident, Incident continued, and Individual Listing.

(1) The Multiple Casualty Listing Form allows the user to create and edit incident information that is applied to multiple casualty reports. The first two tabs are used to enter incident information and the third tab lists all the casualties that are connected to the selected multiple casualty incident.

(2) Completing the Incident tabs creates a multiple casualty template to which individual casualties are assigned. The template is data that is common to all personnel involved in the incident. For example, if this is a joint operation, the “Service” drop-down would not be used.

(3) Creating a template for the multiple casualty incident reduces the time involved creating each individual casualty report with the same common data. Errors are also reduced in the critical data fields such as date and time of incident, location and circumstances. These fields should be the same for all personnel involved.

p. The Contingency CAC may assign Codes to prevent duplicates and to maintain a consistent overall Contingency Operation count. CMAOC will manage the multiple casualties as a group, ensuring that the NOK of the deceased casualty is notified first, then the wounded casualty NOK. All notifications must be complete for 24 hours before release of information. Complete SUPP, PROG, or STACH reports as needed for each individual casualty. Casualties later determined to be part of a multiple casualty are easily added by selecting the “multiple casualty code” drop-down menu on the casualty screen and selecting the multiple casualty template previously created describing the incident. High visibility and joint force casualties must be coordinated carefully and quickly to ensure swift and complete NOK notification and news release.

NOTE: Tell the students that when they use a multiple casualty incident on a Hasty Report, only the data elements on the Hasty are used regardless of whether or not they entered the information on the multiple casualty incident. For example, vehicle information can be entered on the multiple casualty incident; however, the Hasty Report will not use this information. The Full Report will use all the multiple casualty incident information.

45. DCIPS-CR

NOTE: Outside of the training environment, this is how the DCIPS Log-in screen appears.

a. As described earlier in the lesson, DCIPS-CR (Casualty Reporting) is a web-based application used by units and higher echelons to report information higher. It imports hasty or full reports from DCIPS-CF. DCIPS-CR provides secure reporting capabilities for casualty reporting and provides greater flexibility and more visibility of the casualty data. Users must have a CAC to use this site. To enter the DCIPS-CR Portal, open a web browser.

1. Enter the following URL - https://dcse.hrc.army.mil

2. The Digital Certificate window will open; select a non-Email certificate with the user's name and click “OK”.

3. For new users, click on “Account Request” and fill out the application which already recognizes the user's CAC. The application is reviewed by the DCIPS Portal Administrator and notification is sent when the account is established. Ensure the application is complete before submitting it to the Portal Administrator.

4. For existing users, click on “LOGIN”. The login screen for user name/password will display; enter the information and select “Register”. Once complete, the user's CAC will be linked to the account. The next time the user logs in with the CAC, there will be no requirement to enter the username/password.

5. Select the “OK” button. This action will cause the DOD Notice and Consent window to open.

6. Select the “I Agree” button (or select “Close Browser” button, if you choose to quit the process).

46. DCS Portal - Main Interface

b. To begin the DCIPS-CR reporting process, log-on to the Portal using the CACenabled log-on and select DCIPS-CR Forward from the Application Listings section.

NOTE: Tell the students to check the Army and DoD Message Boards each time theylogin for database and general message updates.

NOTE: Inform the students that they will only see the applications to which thehave access.

47. Summary/Home Page (1 of 2)

NOTE: While most of the requirements are the same, the look of the CR screens is a little different from CF, and you have more options.

c. On the main DCIPS-CR home page, select a “Reporting Unit” from the drop down menu if there is not a unit automatically displayed. Normally, users are assigned one Reporting unit, and possibly more, if necessary. Next, on the Summary/Home page, Select a Reporting Unit”.

d. After selecting a reporting unit, a summary of reports are displayed detailing the types of reports in the queues. Only queues with reports are displayed.

(1) Summary For Reporting Unit (which report to you).
(2) Processed By Service HQ.
(3) Rejected (by higher levels).

e. Users can also select an action from the Menu at the top of the Summary/Home Page:

(1) New Casualty – The initial report on a new casualty.

(2) Import – User can import data from DCIPS-CF into DCIPS-CR by saving the report to a file.

(3) Multi-Casualty Template – Used to report two or more casualties of the same incident.

(4) Multi-Casualty Assignment – Where the incident occurred and what type of incident.

(5) Reports - Reports that the user has created.

f. The “Show Filter” link displays the date range of the reports in the queue. It is meant to reduce the amount of records displayed in the queue to a manageable level. Any changes to the date range will stay in effect for the duration of a session. Once the user logs out of DCIPS-CR, the date range will return to the default range.

49. Queue Page

g. From the Summary/Home Page, click on “Queue”. The Queue Page displays reports created when selecting a Reporting Unit. It displays only the reports in aselected queue on the Summary/Home Page. The queue selected will be ‘bolded'.

Action buttons are located at the bottom of the Queue Page and can be located in the Help Menu. The queue can be filtered to list only the records that are of interest to you.  

(1) New - Initial report.

(2) Edit - Report can be edited as long as the user hasn't submitted it.

(3) Replicate - Creates a supplemental report.

(4) Submit - Sends the report forward.

(5) View Related - Used to view all related reports pertaining to a specific casualty.

(6) Report - Used to review existing casualty reports.

(7) Send Copy - Used to send a copy of the report to others.

(8) Remarks - Used to post remarks to the report.

(9) Delete - Used to delete a report.

(10) Reject - Shows reports you submitted that were rejected by the next higher authority.

(11) Export (Excel) - Used to export a file.

(12) Resubmit - Used to resubmit a report that has been rejected.

50. Queue Page Filter

h. The filter options are hidden by default.  You can show the filter by selecting the Show Filter link.  To perform a search, select the items to be filtered and press the “Filter” button.   The Queue will refresh to show only those records matching your filter search criteria.  Select the Hide filter link to hide the filter criteria.

51. Creating a New Casualty Report

i. To create a new casualty report, select “New Casualty” from the Menu on the Summary/Home page. Enter SSN for Personnel Database search; enter SSN and initials for a Defense Manpower Data Center (DMDC) search. Search results shown will be:

(1) Matching names found in personnel database.

(2) Matching casualties found in DCIPS database.

(3) Matching records found in DMDC.

(4) No matches.

j. The personnel database information is automatically downloaded from the ITAPDB whichpopulates certain data fields on the report. The DMDC search will provide some personnel Data; but has no NOK information.

k. Choosing “Select” will move the user to a new report screen.

l. The DCIPS database indicates if there is already a record for the Soldier in DCIPS. This could be a different incident or the same incident. Compare the incident dates and select if the same incident. This action will prompt a pop-up stating “You are about to create a supplemental report for the selected DCIPS record. Continue?” Supplemental reports will be covered in another section.

m. “No matches” indicate no records found; select the “New Casualty” button to move to a new report screen.

52. New Casualty

NOTE: Tell the students that this is an example of the search results page.

n. You have two options before starting your new casualty report. You can use the ITAPDB to pull in personnel information (if a military casualty) or use the DMDC to pull in basic Soldier and NOK information from DEERS. You do not have the option to pull from both data bases. Recommend you pull from DEERS unless you have all of the confirmed NOK information available to you. While ITAPDB provides some personal information, it will NOT provide any NOK information.

o. Select the DMDC Search button. If the casualty you're trying to report is in DEERS, the system will advise you that a Matching DMDC Record was found and then provide you with an option to Select that file for input into an Initial (INIT) report. When you click on the Select option, there will be a “pop-up” asking if you wish to continue. Click OK.

53. Casualty Report

p. When creating an Initial Report, the Casualty Report page is the “home” page for the report. All other pages are sub pages and will display their title next to the Casualty Report Title. Work through the drop down menus to create a report or change data for supplemental reports. When complete, select “Update” or “Update and exit” from the action buttons located at the bottom of the screen. “Mark as Ready” enables the report to be submitted to next higher level. The Casualty Page is a hasty report which includes the place of incident in the circumstances; this is all that is needed for a Theater casualty report.

q. The Multiple Casualty Code data field has a drop down menu to select a Multiple casualty incident template already created. Selecting a template will overwrite anything previously entered in specified fields of the template.

r. Theater Reporting - The Army requires Body Armor to be added to a casualty report and has a separate page for that purpose. An initial report for the Army is the Casualty Report page and the Body Armor page. Reports may also include diagnosis for wounded personnel on the Progress page.

s. Click on “Show DCIPS” to view the DCIPS side by side screen. This displays current information in DCIPS from previous reports.

54. Scenario New Casualty

NOTE: Have the students use the DD93/SGLI for Caylor, Kirk (DCIPS Supplement to LP) to create a new casualty report. Have the students complete only the Casualty and Body Armor. Have the students “Update” the record.

CIRCUMSTANCES FOR VSI ill/Injury SOLDIER:
SPC Caylor, Kirk., 901144601, and his platoon were in the vicinity of MC 2153671253 when the vehicle struck an IED. SPC Caylor, a passenger, sustained a serious laceration to the right leg and serious head trauma. Verified by MAJ Michael, Lawson, MD, 28TH CSH. Soldier was air evacuated to 28th CSH. This information was verified by MAJ Rivera, Alexis, BN S3. Soldier is awaiting evacuation to Landstuhl Hospital, Germany.

REMARKS FOR SI SOLDIER:

55. Progress Report

NOTE: From the Home page, Click “Queue” click the radio button for Caylor, Kirk click “Edit” from the Casualty Report Page, Click “Progress Report List”

t. The purpose of a Progress Report (PROG) is to provide the NOK with updated medical information on the patient's condition that was received after the INIT report was submitted, prognosis and any new diagnosis. Each PROG will contain:

1. The patient's morale, state of consciousness and mental disposition.

2. Whether he or she is ambulatory and or taking nourishment.

3. Current and anticipated treatments to include surgical procedures (type and whether or not successful).

4. Additional and terminal diagnosis.

5. Anticipated period of hospitalization.

6. Any evacuation plans, including dates and destination.

7. Name and commercial phone number of the attending physician, if available.

8. Whether or not an overseas command has issued a transportation and travel order (T&TO) to the NOK; and if so, each PROG will indicate whether the NOK is at the bedside or has departed.

9. Any evacuation plans.

10. Include any information that might reduce NOK anxiety. For example, state of consciousness and mental disposition; degree of alertness; whether or not the patient is taking nourishment; whether the patient is ambulatory; type of diet; whether or not the patient is on life support equipment (type of life supportequipment and medical status).

u. You cannot submit a PROG until after the INIT has been submitted. Each PROG report will deal with a single patient and will be sequentially numbered for control purposes (PROG08, PROG09, etc.) Contact the MTF for update of medical status. Even if the individual's progress remains the same, contact the Medical Treatment Facility (MTF) and submit PROG reports at least every 24 hours for VSI and SI until a family member arrives at the bedside or the casualty is discharged.

56. Scenario Progress Report

NOTE: Have the student complete a progress report from the scenario. Assume the initial casualty report has been successfully submitted to higher.

Today's Date: SPC Caylor, Kirk has been stabilized and is en route to Landstuhl, Germany. He is alert and responding to verbal commands. He is receiving intravenous fluids due to the seriousness of the head lacerations. He will undergo surgery at Landstuhl to repair several ligaments in right leg. He will undergo eye surgery to repair a torn retina in his right eye. USAREUR G1 Casualty has issued travel orders for his wife and is awaiting passport.

Will SUPP when more information becomes available. Responsible CAC: USAREUR G1 Casualty. POC for this report is SFC Wall, Tommy T., MND-B Casualty Ops @ 318-847-1864.

57. Multiple Casualty Template and Assignment

v. A Multiple Casualty Template is used to create a multiple casualty code and multiple casualty description for incidents that involve two or more Soldiers. A Multiple Casualty Assignment is used to assign previous casualties to an incident. Both can be selected from the Menu on the Summary/Home Page.

w. Apply the Multiple Casualty Template to several casualty reports.

1. Select the Multi Casualty Assignment link from the menu on the Home page.

2. Select the incident from the Select Incident drop down combo box.

3. Select the casualty reports  to apply the template.

4. Press the Assign push button.  The information in the selected incident template will overwrite the corresponding fields in the selected casualty reports.

5. Return to the Queue, select the case and complete any updates for submission.

NOTE: Using a Multiple Casualty Template reduces the chance of reporting error.

58. Multiple Casualty Template

x. Click on “New Incident Template” to create a template with uniform information about an incident involving two or more Soldiers.

y. Apply Template to individual case records from the Casualty Page

1. From the Queue Page, select a record and press the Edit push button.

2. From the Casualty Page, select the Multiple Casualty Code drop down.

3. Select the multiple casualty template from the Multi Casualty drop down. On an Initial casualty report, selecting a multiple casualty template will populate the specific incident fields with the template information.  On a Supplemental report,  the current incident data will be replaced with the template data. This new information will overwrite current data in the related fields on the Casualty and Incident pages.

4. A pop-up will ask if it is “OK” to load the new data.  Select “OK” or “cancel”.

5. Complete any other updates to the casualty report for submission.

6. Scroll to the bottom of the screen, select “update” or “update and exit”.

59. Creating a New Template

z. To create a New Incident Template, fill in the data fields with incident information which is specific to the INCIDENT and is the same for all casualties involved.

1. Individual information will be added to each individual report.

2. Select “Add”; will return to the template listing.

3. Next, create casualty report for the individuals involved.

4. From the template listing, select the template to work with.

aa. Create a Multiple Casualty Template by following these steps:  

1. From the Home page select the “Multi Casualty Template” link from the Menu at the top of the page.  A list of existing multiple casualty templates will display.

2. Press the New Incident Template push button.  The Multiple Casualty Template page will display.

3. Create a multiple casualty code and enter it in the Multi Casualty Code field.

4. Create a title or description of the multi casualty incident and enter  in the Multi Casualty Description field. Complete the remaining information common to all casualties that are part of the incident.

5. Press the Add push button.  You will be returned to the list of Multiple Casualty Templates.

60. Multiple Casualty Assignment

bb. Use the Multiple Casualty Assignment screen to assign casualties previously created, but not yet submitted. If a report has been submitted and later found to be part of a multiple casualty:

1. Replicate the report for a SUPP and select the Multi Casualty Code from the drop-down menu on the Casualty Screen.

2. Assign from edit status using Multi Casualty Assignment.

NOTE: Inform the students that clicking on the “ASSIGN” button will assign a multiple casualty number and clicking on the “CANCEL” button will simply cancel the report.

61. Submission Screen

cc. To submit a casualty report, select “Queue” from the Menu on the Summary/Home page. Only reports in “the Ready”' status may be submitted. Default submission goes to higher HQ as stated in hierarchy. Additional reports may be sent to reporting units the user wishes to notify by checking the boxes on the distribution list (DCIPS User List). This will generate an Information copy which may be replicated for supplemental reports. Once reporting units have been selected, click on “Confirm Submission of Casualty Report and Notification of Reporting Units” to submit casualty report.

dd. An abbreviated email report may be sent to those who do not have DCIPS, but have a need for basic information on a casualty. This email report may not be imported to any DCIPS application. To use this feature, click on the “Send Casualty Report” link located at the bottom of the screen.

ee. Users may create a text copy of the report which may be saved and attached or copied to an email. This report also cannot be imported to any DCIPS application. To use this feature, click on the “Download Casualty Report” link located at the bottom of the screen.

62. Send Copy To Screen

ff. From the Queue page, the “Send Copy To” feature allows the user to send an Information copy of a casualty report to anyone in the distribution list. From the receiver's queue, this Information copy may be replicated to create supplemental reports or retained for archive information. To use this feature, click on the “Send Information Copy” link at the bottom of the screen.

gg. For Privacy Act purposes, these reports DO NOT include NOK information. DCIPS users, who need the NOK information, will receive an Information copy to forward to the CNO/CAO for performance of their mission.

NOTE: Tell the students that the “Download Casualty Report” and “Send Casualty Report” links operate in the same manner as previously described.

hh. Download Casualty Report Push Button: This button is only available after you submit the casualty report by pressing the Confirm Casualty Report push button. When you press the Download Casualty Report push button a File Download dialog will open. You can then save the casualty report to a text file or open the casualty report. If you open the report it will open using Notepad.

ii. Send Casualty Report push button: This button is only available after you submit the casualty report by pressing the Confirm Casualty Report push button. When you press the Send Casualty Report push button, Microsoft Outlook will launch and display the casualty report in an email message. You can then add your email addressees to the Outlook message and send as you would any other email.

63. Importing Casualty Reports (1 of 2)

jj. If you are receiving casualty reports created in DCIPS-CF format, you will need to Import the reports into DCIPS-CR before forwarding them on to CMAOC. Take the following steps to Import a casualty report:

1. When you receive a DCIPS-CF casualty report via email, save the report to a specified location on your desktop so that you can retrieve it at random.

2. Go into DCIPS-CR, and click on the Import button. When the screen opens, click on the Browse button and go to the file where you stored the DCIPS-CF report. Select the report you wish to import. You should get a red message stating that the record processed successfully, and you'll also be given the Queue Number.

3.Now click on Queue and select the file that matches the Queue Number/Name you were given for the imported file. Click Edit on the lower left of the screen.

4. Click the button marked Personnel Database Compare. From the right hand column marked Personnel Database, click the left arrows under Replace to add (or replace) personnel information shown in this column to the DCIPS Casualty column.

NOTE: Tell the students that there are currently no external databases that support the Training Database

5. Click Update to save the changes. When finished, click on the Casualty button at the top left. Click on “Update” or “Update and Exit” to complete the Import action. This program will not completely fill the report, but is intended to Validate some of the personnel data, and add some that may be missing.

6. Once you have completed your updates, process the DCIPS-CR report for forwarding to CMAOC and other recipients.
